Demonstrably well organised
Addiction Support works in line with the regional Wmo quality framework of Brabant Noordoost-Oost and the Dutch Care Quality, Complaints and Disputes Act (Wkkgz).
Working area: the Brabant Noordoost-Oost region in the Netherlands (Oss, Bernheze, Boekel, Maashorst and Land van Cuijk) and surroundings. Support is provided on the basis of a municipal social-support (Wmo) decision or a personal care budget (PGB).

If we cannot resolve it together, you may call free of charge on an independent complaints officer, who mediates and takes no side: Klachtenloket Zorg, +31 (0)70 310 53 92 or info@klachtenloket-zorg.nl, weekdays 9:00–15:00.
You will receive a written, reasoned decision within six weeks of submitting your complaint. If more time is needed, you will be informed in writing within those six weeks; the term is then extended by no more than four weeks.
If you disagree with the decision
You may submit the dispute to the Disputes Committee for General Care (Geschillencommissie Zorg Algemeen), to which Addiction Support is affiliated. The committee issues a binding ruling and may award compensation up to € 25,000. A fee applies. Submission must take place within twelve months of the date you filed the complaint.

Then your municipality’s complaints procedure applies. The Disputes Committee has no jurisdiction over Wmo support; that route runs via me, then your municipality and, if needed, the ombudsman. The route above applies to privately funded support and to support financed through a personal budget under the Wlz or Zvw.
Client satisfaction
At least once every two years, Addiction Support surveys client experiences in line with the regional Wmo quality framework — including: how do you rate the quality of support, does it match your needs, and does it improve your self-reliance and quality of life? Improvements are demonstrably acted upon and the results are public.

Working method & quality system
Addiction Support uses a lightweight quality management system fitting a small practice: a Plan-Do-Check-Act cycle, periodic evaluation (at least yearly) of the goals in the care plan (‘trajectplan’), and a fixed procedure for reporting incidents and data breaches.
- Reporting code for domestic violence and child abuse — the statutory step-by-step protocol is demonstrably applied.
- Subcontracting — support is delivered in person. If a third party is engaged by exception, this is reported to the municipality in advance.
- Privacy — client data is processed in accordance with the GDPR; see the privacy statement.
- Care plan (trajectplan) — goals are drawn up together with the client, formulated SMART and evaluated at least yearly.
What we measure and improve
- Client experiences — surveyed at least once every two years, with improvements demonstrably acted upon.
- Intake, progress and exit — mapped annually.
- Early drop-out — analysed annually, with corrective measures where possible.
- Complaints — reported annually to the region by nature and cause, including improvements.
Your rights as a client
- Freedom of choice — you choose your own provider.
- Independent client support — you are entitled to free, independent client support in dealings with your municipality.
- Participation — clients have a say in how support is organised, via periodic evaluations and client consultation.
Safety and cooperation
- Safety risks are mapped at the start, discussed with the client and reviewed at every evaluation.
- Child check — at intake we always check whether children are involved and whether their situation is safe.
- Informal carers — the load on family members is taken into account; we refer on where overload threatens.
- Cooperation across the care chain — where several providers are involved, tasks and case coordination are recorded and aligned at least yearly.
Calamities
Serious calamities and incidents of violence are reported within three working days to the Wmo supervisor of the regional public health service (GGD Hart voor Brabant).
